Who We Are:
Elevate Wellness Center, LLC provides administrative and coordination services for telehealth. We are not a medical practice. We do not diagnose conditions, provide medical treatment, or write prescriptions.
Who Provides Care:
All medical services — including evaluation, diagnosis, and prescriptions — are performed by licensed, independent healthcare providers who comply with the laws of the state where the patient is located.
No Guarantee of Treatment:
Submitting an intake form does not guarantee treatment, medication, or prescriptions. Providers may determine that care is not appropriate or that a prescription is not necessary. Providers may decline care for any reason consistent with their professional judgment.
Patient Responsibilities:
- Provide accurate, current information.
- Use telehealth services responsibly.
- Understand that telehealth is not for emergencies — call 911 if you are in crisis.
State Compliance: Elevate Wellness Center complies with all applicable telehealth laws and regulations. Providers are licensed in the states where patients are located, and follow those state laws when delivering care.
Website Use: This website is for personal, lawful use only. We may modify or suspend access to the site or services at any time.
Intellectual Property: All content, logos, and materials on this site belong to Elevate Wellness Center and may not be copied or distributed without permission.
Limitation of Liability: We provide this website and coordination services “as is” and make no guarantees of uninterrupted service. We are not liable for delays, service interruptions, or outcomes of medical care, which are solely between you and your provider.
