    After you complete your intake, an independent licensed medical provider reviews your information and determines whether treatment is appropriate. When prescribed, medications are filled through certified partner pharmacies and shipped directly to your home.


    Our role is to coordinate your care experience and provide guidance throughout your journey, while all medical decisions are made by the licensed providers we work with. Our goal is to make high-quality wellness care accessible, affordable, and easy to integrate into your life.

  • Step 1: Complete a secure online intake form. Share your health background, goals, and preferences.

    Step 2: A licensed provider reviews your intake asynchronously. In some states, a video or audio visit may be required.

    Step 3: If approved, the provider issues a personalized treatment plan and prescription if appropriate.

    Step 4: Your prescription is filled by a licensed pharmacy and delivered to your door.

    Step 5: We support you with weekly reminders, lifestyle tips, and ongoing messaging-based care coordination.

  • In most states, no. The provider reviews your intake and issues a plan without a scheduled visit. If a live telehealth consultation is required by state law, you'll be prompted to schedule one.

  • No. Submission of an intake form does not guarantee a prescription. A licensed provider assesses eligibility and appropriateness based on clinical judgment.

  • Medications are dispensed by licensed pharmacies in the U.S. and are shipped directly to your address—no pharmacy lines or waiting rooms involved.

  • Yes. Boosters like B12, NAD+, Glutathione, Sermorelin, and GHK-Cu can be added for an additional fee. All require provider approval and a valid prescription.

  • We do not bill insurance directly. However, we accept HSA/FSA payments, and you may be able to submit receipts to your insurer for reimbursement depending on your plan.

  • Your provider remains available through our messaging portal. You can ask about side effects, dosage adjustments, or other questions at any time.

  • Our providers are licensed across multiple U.S. states. Availability of specific treatments and coverage may vary by state.

  • Compounded medications are custom-prepared by licensed pharmacies based on an individual prescription. They may not carry FDA approval but are produced under stringent regulatory standards in FDA-inspected facilities. They are only prescribed when deemed appropriate by your provider.

  • Due to the medical and regulatory nature of prescription treatments, once medication has been dispensed, we cannot offer refunds or returns.

  • Once approved, medications typically ship within 2–5 business days, depending on pharmacy processing times.

  • Elevate Wellness Center does not provide emergency medical care. If you are experiencing a medical emergency, call 911 immediately or go to your nearest emergency room. For urgent but non-emergency questions about your treatment, use our patient portal to contact your prescribing provider.

  • Licensed providers review your intake and prescribe medication only if appropriate.

  • Some medications we discuss, like Ozempic®, Wegovy®, and Mounjaro®, are FDA-approved brand-name medications. Elevate Wellness Center works with licensed providers who may prescribe these medications, or they may prescribe compounded versions made by FDA-registered 503A/503B pharmacies when appropriate. Compounded medications are not FDA-approved, but they are prepared in certified facilities that meet strict quality and safety standards. Whether you receive an FDA-approved medication or a compounded version will depend on your consultation and the provider’s clinical judgment.
