Prescription Policy

Elevate Wellness Center partners with independent, licensed physicians to provide telehealth services. Prescriptions are not guaranteed and will only be issued if clinically appropriate after a full medical review.

• Provider discretion: All prescribing decisions are made solely by the independent provider based on your health history, labs, and clinical judgment.

• No automatic refills: Prescriptions may require re-evaluation before renewal.

• Patient responsibility: You must provide accurate and up-to-date information for safe prescribing.

• Safety first: Providers may decline, change, or discontinue a prescription at any time for medical or safety reasons.

By submitting an intake form, you acknowledge that medications will only be prescribed if deemed appropriate by a licensed provider and that Elevate Wellness Center itself does not prescribe or dispense medication.
