We’re a female- and PA-founded management and wellness consulting company committed to helping you feel more empowered in your health. Our role is to provide personalized support, education, and care coordination services in partnership with licensed medical providers who handle all prescribing and clinical decisions.

OUR FOUNDER

Meet Samantha, PA-C

Samantha is a certified Physician Assistant with a Master of Physician Assistant Studies and over 8 years of experience in the healthcare system. After nearly a decade of working within traditional medicine, she felt called to do things differently; more personally, more holistically, and more in line with helping women feel their best.

A passionate advocate for functional medicine, Sammie founded Elevate Wellness Center to offer the kind of care she’d want for herself or someone she loves—honest, compassionate, evidence-based, and sustainable.

When she’s not supporting patients, you’ll find her out in nature snowboarding, hiking, camping, or soaking up a little too much sun. She also loves international travel, dancing, staying active, and spending time with her family.

  • Elevate Wellness Center is a modern telehealth platform focused on helping you feel and function at your best. We offer personalized support for weight management, hormone balance, peptides, and longevity therapies — all through a simple online process.


    After you complete your intake, an independent licensed medical provider reviews your information and determines whether treatment is appropriate. When prescribed, medications are filled through certified partner pharmacies and shipped directly to your home.


    Our role is to coordinate your care experience and provide guidance throughout your journey, while all medical decisions are made by the licensed providers we work with. Our goal is to make high-quality wellness care accessible, affordable, and easy to integrate into your life.

  • Step 1: Complete a secure online intake form. Share your health background, goals, and preferences. 

    Step 2: A licensed provider reviews your intake. In some states, a video or audio visit may be required. 

    Step 3: If approved, the provider issues a personalized treatment plan and prescription if appropriate. 

    Step 4: Your prescription is filled by a licensed pharmacy and delivered to your door. 

    Step 5: We support you with weekly reminders, lifestyle tips,and ongoing messaging-based care coordination.

  • Should medication be suitable, you can acquire it via the Elevate Wellness Center platform at significantly reduced prices compared to other websites, often even more cost-effective than your neighborhood pharmacy.
